Personal Details
Title:

Gender:
We ask for your gender only so that we can monitor our equal opportunities practices. This piece of information is not used for anything else. You may be aware that women are under-represented in construction trades. On-Site Bristol is keen to redress this imbalance and so we measure whether we recruit women in line with the numbers that apply, to make sure that we are not indirectly discriminating against these applicants.

E-mail:
if you have an e-mail address that you regularly use and are happy for us to send information by e-mail, please give the address clearly and in full.
Age:
please give your age as at the day you are completing the form.
Date of Birth:
please give your date of birth in the format shown on the form, that is in the order day, month, year, using two digits for each. While entry to On-Site Bristol’s Apprenticeship programmes is covered by the provisions of Age Discrimination legislation, the funding we receive does vary according to the age of recruits, when they start an apprenticeship, and so we do need to be quite clear about this.
National Insurance Number:
if you have been given your National Insurance (NI) number, please enter it clearly. You will need to give us you NI number before you start an apprenticeship, so if you do not have it yet, please remember to let as know as soon as you do receive it (usually shortly after your 16th birthday)
Next of Kin:
this could be your parent or guardian; someone we could contact in an emergency or with whom we could leave a message if we can’t contact you
Ethnicity:
As with gender, we ask for your ethnicity only so that we can monitor our equal opportunities practices. This piece of information is not used for anything else; we measure whether we recruit ethnic minorities in line with the numbers that apply, to make sure that we are not indirectly discriminating against these applicants.
Disability:
We ask about any disability, or learning difficulty, partly to monitor equality of opportunity, but also so that we know if we need to make special arrangements for testing and interview. We quite often receive applications from young people with dyslexia, for whom we will organise readers, or other assistance, to give them an equal chance when it comes to taking the tests. Another example might be of an applicant with impaired hearing, for whom we can organise a signer, if appropriate.
Education
Please give full details of
- The last school (or college) that you attended for full-time education.
- The date when you are due to leave or when you did leave that school (or college).
- The subjects in which you are taking, or have taken, examinations. These are usually GCSE’s, but may be other qualifications such as GNVQ’s.
- If you haven’t got your grades yet, please enter the Predicted Grades for each subject.
Previous Training:
if you have been on an Apprenticeship, or E2E, before, please give the name of your training provider and the number of weeks that you were with them.
Choice of trade:
if you are interested in just one trade, please tick the appropriate box.
if you would consider two or more trades, please indicate which is your favourite, using number 1 for first choice, 2 for second, and so on.
Work Experience:
this does not have to be construction related; it could include work experience from school, a part-time job or work that you have done for family or friends. If you have done some construction related work experience, please give the company name clearly and in full.
Interests:
please give a brief description of any hobbies or sporting activities, either organised or informal; generally, what do you do with your free time?
Why construction:
What is it about construction, and your chosen trade in particular, that attracts you? Is it working with your hands, actually seeing something that you’ve worked on, because a family member is in the trade, the chance to travel around or something else?
